According to a report by the National Library on Medicine:
"Homelessness is associated with premature morbidity and mortality at much higher rates compared to those who are housed. Healthcare for high-need patients experiencing homelessness is often inadequate with many studies finding high rates of acute healthcare utilization,decreased utilization of primary care, and poor quality outcomes observed in this population. The care provided to persons experiencing homelessness is often fragmented and reactive to acute presentations.
Primary care, with its longitudinal, comprehensive, relationship-based model, may provide a platform for improved care and outcomes among those experiencing homelessness. However, traditionally structured primary care models often do not meet the needs of socially vulnerable patients experiencing homelessness due to issues related to complex healthcare and social needs, lack of trust, stigma around seeking treatment, and disrupted care processes."
